1) download Deathball 2.0
2) Install the files in UT2004/Deathball
3) get the newest files
4) overwrite them
5) Double-click (that's 2 times (!!)) Deathball.cmd
6) PLAY

Your step 2 could be taken the wrong way.
=====================================
INSTALLATION FOR DUMMIES
(yes we know you're not stupid)
1) Download the full Deathball 2.0 beta zip AND the latest beta update
2) Extract the full Deathball 2.0 beta zip to the main UT2004 directory (example: C:\UT2004)
3) Extract the beta update to the same directory (example: C:\UT2004)
Say YES if prompted to overwrite any existing files
4) Double-click on the file "Play Deathball.cmd" found in UT2004\deathball to start the game
-=IMPORTANT=-
Should the game indicate a problem with your CD-Key at startup:
Navigate to UT2004\deathball\system
and delete the "Deathball.ini" and "DeathballUser.ini" files
The problem should now be resolved
=====================================
Do you think that's fool proof enough?

Try this.
Backup your
\ut2004\system\ut2004.ini
\ut2004\system\user.ini
Delete
\ut2004\system\ut2004.ini
\ut2004\system\user.ini
\ut2004\deathball\system\deathball.ini
\ut2004\deathball\system\deathballUser.ini
If it's not still working, try following.
1. Click Start -> "run..." -> type "cmd"
2. In the prompt, type "cd \ut2004"
(Or whatever your ut2004 dir is)
3. type "dir /s > list.txt"
4. type "notepad list.txt"
5. Copy paste everything to me via medium you like...

There's also an 'Edit' button, mostly used for 'editting'.

JoshuaC, some of your ut2004 files are missing. E.g. Ut2004\System\Default.ini - This is must to have. Please reinstall ut2004.
