All it takes is a fair and objective, mature admin/regular player on the server. Period. Someone with integrity and trusted judgement that will try to correct the problem in a polite way in progressive steps with the minimum action necessary.
Look, mistakes happen, some people can't get along, and then there's those that have no intention on playing nice no matter what is said to them and however many chances they are given. With me, if a ban can't be justified, it won't stand. It will also piss me off that an admin I trust to do the right thing is making bad calls. I can't tell you how much I hate seeing crap like this: "Player-X is a fag. He's banned"..
Self goaling though.. if it's intentional and characteristic of a player, its a jerk that doesn't want others to enjoy your server. Hammer time for this clown. If he wants to repent, my email is in the server banner for them to explain themselves.. Otherwise the on the spot apology is always good.
