You randomly select patients either to receive the treatment or a placebo. Ideally it should also be double blinded, meaning neither the patient nor the person treating them knows who's getting the real treatment. Then you look to see if there's a statistically significant difference in how well the patients do.

No, ethically you have to give them the most effective known treatment, rather than a placabo.
